    3.  Supplemental applications (at the underwriter's discretion).
    4.  Copies of the underlying GL and AL quotes

    To Bind Coverage:
    1.  A written request to bind;
    2.  A thorough Acord submission;
    3.  At least 5 years of loss runs for all underlying policies;
